I've just installed Small Business Server 2003 Premium Edition and fully patched it, but now the Add User Wizard crashes.  The information window loads, but the wizard crashes as soon a I click Next.
 
The add_user_wizard.log has no useful information.

Interesting I was able to run the Internet Connection Wizard yesterday (before applying all the patches) but today it is crashing in exactly the same fashion...
Faulting application icw.exe, version 5.2.2651.0, faulting module unknown, version 0.0.0.0, fault address 0x000a3240.
 
Note - I have also installed a Windows Server 2003 Standard Edition as a second domain controller.
 
Has anyone seen this before?  What can I do?
 
I have successfully added a user through Active Directory Users and Computers, but I'd rather use the Wizard.
 
Any advice much appreciated.  Thanks!

More information -
Upon reboot I received a multitude of error messages reporting that DEP had prevented the Add User (or Internet Connection) Wizard from running.
So I disabled DEP for the Add User Wizard...and it works!

Problem solved (hopefully).

One more note for anyone else reading this -
I'd only installed the automatic patches through Microsoft Update. I hadn't manually installed Small Business Server Service Pack 1.

You should format the HD and start over. Apply the SP1 first. Then see how it goes.
