I have followed this artilce identically and on the client's computer I still get a message saying that I do 'not' have permission to send with this account.  any help?

I haven't read the article, however this is quiet simple.
Create a new user, with mailbox ofcourse - use the add user wizard.
Then open this users properties in AD user and computers.
On the EXCHANGE GENERAL tab, click DELIVERY OPTIONS button
Then in the Send on behalf of, grant this perimssion to box add the users whoyou wish to send on behalf of this user.
You may have to wait a while after granting the send of behalf perission to update on the server.
Then on the Outlook of the people how are to send on belf of, add the FROM button.
User click the from buttom, choose the new user account when send emails.

Did you close and open Outlook after adding the other user's mailbox?
