I have followed all procedures and checked against other working sites the Outlook over Internet solution (RPC over HTTPS). However the connection keeps getting refused. I have checked the ISA logging and the following is logged, I can only assume their is an issue with the DNS zone domain.com in the DNS I have created to allow RPC to work both internally and externally or alternatively with the Certificate however both the ICW and certificates have been recently run and certificates installed.
 
The issue as logged in ISA is as follows (I have changed the domain names to domain.com and domain.local):

I would delete that DNS zone that you have added and try to get Outlook over the Internet working from outside first.

Thanks for your reply on this, I have very small windows of opportunity to make change or test at that site due to remote users on various time zones. The DNS Zone is in place as they also have mobile users randomly needing resources such as OWA both inside and outside depending where they are. I will delete the DNS zone and test again from an external source early next week, would you think I need to rerun the ICW...it has only recently been run however I was suspicous of the certificate being valid, I installed the certificate following the last ICW.

I don't think you will need to rerun CEICW, and if you still want to rerun it, don't replace the web certificate (assuming that it is good). After deleting the dns zone, I would empty the DNS cache and restart DNS server at least.
