I'm sitting behind ISA 2004 and trying to get to ftp://ftp.domainname.com.  No problem when not behind ISA, but no connections behind ISA in the domain.  Already unchecked the Read Only boxes on four ISA Firewall Policies and opened port 21for Authenticated Users.  What do I need to configure to pass through ISA?

I doubt its your ISA....more than likely its your router not liking an internal client trying to connect to the external address....try FTP://servername or //serverIP instead while internal....

Which four ISA firewall policies?? You only need to uncheck the Read Only on the properties of the SBS Internet Access Rule and ofcourse have the ISA firewall client installed on the workstations. You also don't need to open port 21 at all, and I sure hope you haven't opened that inbound.
